Appearance of Pompholyx

The rash of dyshidrotic dermatitis (pompholyx) occurs on the hands and/or feet. The majority of cases involve the palms and sides of the fingers, but the soles of the feet can also be involved. The rash looks like crops of clear, deep-seated, tapioca-like vesicles and is very itchy. The blisters (vesicles) normally resolve in three to four weeks and are replaced by 1mm to 3mm rings of scale.

People have described the tiny blisters (vesicles) deep in the skin, as looking like sago grains or frog-spawn.

The first stage shows tiny blisters (vesicles) deep in the skin of the palms, fingers, instep or toes. The blisters are often intensely itchy or have a burning feeling. The condition may be mild with only a little peeling, or very severe with big blisters and cracks which prevent work.

The blisters tend to erupt on the sides of the fingers and palms, and often on the dorsal aspect of the distal fingers, where the skin is similar to that of the palms because of the absence of hair follicles.

The later and more chronic stage shows more peeling, cracking, or crusting. Then the skin heals up, or the blistering may start again. One site may be blistering, while another is dry and cracked.

Severe pompholyx around the nail folds may cause nail dystrophy, resulting in irregular ridges and chronic paronychia (nail fold swelling).
